- AW8P- Application form and guidance notes to claim deferred benefits. Please note that you should complete the census form below with your application. Please also read Booklet R before completing this form. If you live abroad please read the bank payments overseas factsheet and complete the bank mandate form applicable to the country your bank account is held in. The bank mandate forms can be found by clicking here.
- Census Form
- RF12 (PFD 49KB): Refund of contributions – use for all staff. Criteria for claiming a refund: the member must be under normal scheme retirement age, have ceased membership of the Scheme and have less than 2 calendar years membership.
- AW158 (PDF): Application form for child allowance where pensionable scheme membership ended after 1/4/2008. Please read Booklet R before completing this form.
- AW158-pre 1/4/2008 (PDF): Application form for child allowance where pensionable scheme membership ended before 1/4/2008. Please read Booklet R before completing this form.
- AW8PC (PDF 216KB) - Application form for Pension Credit Members to claim benefits awarded to them by a Pension Sharing Order. Please also read Booklet D before completing this form.
- AW240 - Application for consideration of entitlement for early payment of deferred benefits due to ill health.
- AP1 - Election to purchase Additional Pension
- AW341 (PDF 37KB): A form to commute ill-health retirement benefits for members who are terminally ill. This form must be attached to your retirement application form. (AW8 or AW8P)
- AW341a (PDF 131KB): An application to commute age retirement benefits for members who are terminally ill and over the normal pension age. This form must be attached to your retirement application form. (AW8 or AW8P)
- SM R9 App - An Application form and notes for members to apply for Protection of Pay or Voluntary Protection of Pay.
- DB1 - Death Benefit Nomination for membership that ended before 1 April 2008
- DB2 - Death Benefit Nomination for membership that started or continued from 1 April 2008
- PN1 - Non registered civil partner/married partner nomination
- NOM1- Cancelling an existing Nomination
