Change of address

Application Forms

AW8P- Application to claim deferred benefits. Please note that you should complete the census form below with your application. Please also read Booklet R before completing this form. If you live abroad please read the bank payments overseas letter and complete the bank mandate form applicable to the country your bank account is held in. The bank mandate forms can be found by clicking here.
Census Form
RF12 (PFD 49KB): Refund of contributions – use for all staff. Criteria for claiming a refund: the member must be under normal scheme retirement age, have ceased membership of the Scheme and have less than 2 calendar years membership.

AW8PC (PDF 216KB) - Application form for Pension Credit Members to claim benefits awarded to them by a Pension Sharing Order.
