• AW8P- Application to claim deferred benefits
• RF12 (PFD 49KB): Refund of contributions – use for all staff. Criteria for claiming a refund: the member must be under normal scheme retirement age, have ceased membership of the Scheme and have less than 2 calendar years membership.

 

Nomination Forms

• DB1 - Death Benefit Nomination before April 2008
• DB2 - Death Benefit Nomination after April 2008
• PN1 - Non married partner Nomination
