On 1 April 2015 a new NHS Pension Scheme was introduced which covers all transitioning and new NHS employees. Some members are entitled to remain in the 1995 or 2008 Sections until retirement through the Scheme’s Protection arrangements. A series of forms are provided for you to nominate someone to receive a survivor pension / life assurance Lump sum or cancel a previous nomination in the event of your death.
Please read the guidance attached to each form before completion and submission to NHS Pensions.
• DB1- Death Benefit Nomination for membership that ended before 1 April 2008.
• DB2 - Death Benefit Nomination for membership that started or continued from 1 April 2008.
• PN1 - Non registered civil partner / married partner nomination.
• NOM1 - Cancelling an existing nomination.
There are no relevant forms for the employer in this section.
