For Primary Care Trust and Hospital Trust requests: In order to complete the form you will be required to provide certain information which is available from ePACT.net. Please find below step-by-step instructions on retrieving this information via the "Prescriber Dispenser Report".
Prescribers who require a prescription search should complete the Primary Care Trust form and forward it to their Primary Care Trust to arrange this request.
