- date of birth
- postcode
We will use the information you enter to tell you if:
- you have a valid exemption certificate
- your exemption certificate expires within the next month
- your exemption certificate entitles you to free NHS prescriptions
- your exemption certificate entitles you to free NHS dental check-ups and treatment
- your age entitles you to free NHS prescriptions and NHS dental treatment
We only hold the information you enter for 30 minutes after you close your browser.
