Member contribution changes on GP forms
To coincide with the changes to member contribution rates from the 1 October 2022, we have published a number of new GP forms for use from the 1 October. These include:

New GP Locum Form A, Form B, and Solo forms are available on the Information for practitioners and non-GP providers section of the Member Hub. The forms should be used to record work undertaken between the 1 April and 30 September 2023.
Freelance GP Forms
The guidance on the freelance GP locum forms A and B has been amended to allow for a relaxation of the ’10 week’ rule in year 2020/21 to take account of the current pandemic.
