A new NHS dashboard went live on Wednesday 5 November 2025, to help Integrated Care Boards (ICBs) tackle potentially hazardous prescribing and reduce preventable harm to patients.
The Patient Harms dashboard, developed by both the NHS Business Services Authority (NHSBSA) and NHS England, gives ICBs the data they need to identify patients at risk of severe harm from their medications and take action to protect them. By identifying at-risk patients, healthcare teams can intervene early and ensure that patients are taking the right medications at the correct doses.
The dashboard is designed to support a reduction in unplanned hospital admissions linked to hazardous prescribing. Recent NHS data shows that admissions associated with potentially hazardous prescribing cost the NHS £19.7 million in the first quarter of 2025/26.
The new dashboard comes after the NHSBSA decommissioned the previous Medication Safety dashboard in April 2025.
The launch is part of wider NHS efforts to tackle over-prescribing and deliver more effective population health management across England.
