Under General Ophthalmic Services Regulations 2008, all contractors are required to complete and submit an annual form providing the number of written NHS complaints received for General Ophthalmic Services (GOS) in their practice.
As last year’s annual collection was paused due to COVID-19, GOS contractors are required to submit complaints received between 1 April 2019 and Wednesday 31 March 2021.
GOS contractors whose email address is held by us will receive their own unique URL to access the survey. Contractors’ unique URL will take them to a survey page prepopulated with their practice’s ODS code and address.
Contractors who have not received an email from us can fill in the survey online. Completing the survey using this link will not include any prepopulated fields.
GOS contractors have until 11:59pm on Friday 3 September 2021 to complete the survey. Contractors with zero complaints are still required to submit a return.
