National COVID-19 vaccination status survey

On behalf of NHS England and NHS Improvement (NHSEI), we are collecting data on the number of dental staff who have already received two doses of the COVID-19 vaccine, intend to have received two doses of the COVID-19 vaccine by 31 March, or are not vaccinated (including medical exemptions).

The information you submit will help NHSEI to:

  • understand the uptake of the COVID-19 vaccination
  • inform operational plans in primary care settings

NHSEI is asking all dental contractors for their support in completing the survey by 11:59pm on Friday 28 January 2022.

Guidance to help you complete the survey 

If you have the relevant information to hand, the survey should take you less than five minutes to complete. 

You’ll need to enter your contract number to access the survey. This will pre-populate a practice address associated with your contract. 

You will also be asked to provide your name and email address at the end of the survey. This is so a summary of your response can be emailed to you once submitted.

Your response based on the actual number of people working on your contract rather than whole time equivalents (WTE). This is to ensure that the reported figures accurately reflect the number of staff involved in primary care service delivery.
