Recording patients' NHS numbers in England

As we move towards a population health and care system, it is important that dental practices ask new and existing patients for their NHS number. While this information will not affect the FP17 submission, NHS numbers help to identify patients correctly and accurately link their details to their oral health records.

Please ask your patients for their NHS number, and if they do not know it, they can find it through these options:

  • Find your NHS number service - your patients will be asked for their name, date of birth and postcode. They will then be sent a text, email or letter with their NHS number. 
  • NHS App - your patients can view their NHS number using the NHS App online on a desktop, a laptop, or by using the NHS App on their smartphone or tablet. 
  • NHS documents - your patients can find their NHS number on any letter or document they have received from the NHS, including test results and hospital referral or appointment letters. They can also find their NHS number on a prescription.
