Under General Ophthalmic Services Regulations 2008, you’re required to submit an annual form providing the number of written NHS complaints your practice has received.
As last year’s annual collection was paused due to coronavirus (COVID-19), you need to submit complaints received between 1 April 2019 and Wednesday 31 March 2021.
How to submit your GOS complaints return
If we hold your email address, you will have received an email from email@example.com. This email contains your unique link to access this survey.
If you have not received an email from us, you still need to complete the return. You can complete the GOS complaints return online.
You have until 11:59pm on Friday 3 September 2021 to complete your return.
