General Ophthalmic Services (GOS) contract applications

Contractors not in the London region POC​​​​​

This information is only for ophthalmic contractors within the London region POC.

If you are outside this region, you should contact your regional team for help with your application.

To deliver ophthalmic services on behalf of the NHS, you need to apply for a General Ophthalmic Services (GOS) contract. 

We're running a proof of concept (POC) in the London region to help standardise the processes involved in applying for a GOS contract. If you're an existing GOS contractor, you do not need to reapply.

GOS contract applications

    Our POC started on 16 September 2019 for contractors based in the London region.

    It covers GOS contract:

    • applications for an individual or partnership
    • applications for a corporate body
    • applications for a dispensing-only practice
    • terminations

    If you're based in the London region, you need to complete the relevant application forms for your organisation. 

    You can find help for completing these forms in Guidance for Applicants (Word: 245KB), and also find examples of completed forms in the relevant business section.

    Dispensing-only Practices

    If you are applying to become a dispensing-only practice, download the Dispensing-Only Practice Form (Word: 237KB) and the bank mandate form (PDF:431KB).

    Complete the forms and return them to nhsbsa.pao-contractadmin@nhs.net.

    Contract Terminations

    Under the terms of the GOS contract, you can request to terminate your contract. 

    Your contract will be terminated 3 months after you serve notice, or at a mutually agreed date.

    If you are part of the London region POC and want to terminate your contract, download the Termination form (Word: 256KB).

    Complete the form and send it to nhsbsa.pao-contractadmin@nhs.net.

    Contract Variations

    If you are based in London and want to vary your General Ophthalmic Services (GOS) contract, you need to contact us.

    If you want to deliver ophthalmic services at a different or additional address, you need to download and complete the contract variation form (Word: 35.8KB)

    You should return the form to nhsbsa.pao-contractadmin@nhs.net.

    We are also processing the following variations:

    • relocations – mandatory
    • change of address – domiciliary practice
    • adding a new borough for a domiciliary provider
    • adding additional premises to a mandatory contract
    • changes in company name
    • change from an individual to a partnership
    • partner joining or leaving a partnership (not to or from an individual)
    • change from a partnership to an individual
    • addition of directors to a body corporate
    • change to or from a body corporate
    • change in GOS hours
    • change of trading name
    • change in banking details
    • change of authorised signatories

    If you want to make any changes listed above, then please email nhsbsa.pao-contractadmin@nhs.net with:

    • the change(s) you want to make
    • supporting information (for example, if it’s a change in GOS opening hours, provide us with the old opening hours as well as the opening hours you want to change to)
    • your current trading address
    • your contact number

    Force majeure event

    If you want to give us notice of a force majeure event, download and complete the preliminary notice of force majeure event form (Word: 29.6KB).

    You need to send the completed form to nhsbsa.pao-contractadmin@nhs.net stating ‘force majeure event’ as the email subject.
