General Ophthalmic Services (GOS) contract applications

Information for contractors not based in London or the South East region

The information on this page is only relevant to ophthalmic contractors based in London and the South East region (Kent, Surrey, Sussex, Oxfordshire, Berkshire, Buckinghamshire, Hampshire and the Isle of Wight). 

If you are not based in London or the South East region, you should contact your regional team for help with your application.

General information

We're running a POC for contractors based in London and the South East region to help standardise the processes involved in applying for a GOS contract. If you're an existing GOS contractor, you do not need to reapply.

We've collaborated with the London Regional Team to ensure contractors are well supported throughout the administration process. The London Regional Team has produced a GOS Contract Application Process and Declaration form (Word: 52KB). Complete the form and submit it to nhsbsa.pao-contractadmin@nhs.net alongside:

  • your new GOS contract application form
  • any supporting evidence

Site visits

If you've already submitted your application and we've organised a site visit appointment with you, complete the site visit checklist form (Word: 107KB). This will save you time during your site visit appointment.

For applicants in the London Region, instances of ‘Change of Ownership’ cannot be progressed to a site visit if we have not received the termination form from the previous contractor. Termination forms can be downloaded from the contract terminations section of this page. To progress your new application make sure any additional documentation required, in this case the termination form to accompany the new application, is submitted to us as quickly as possible.

Apply for your GOS contract 

Dispensing-only Practices

If you are applying to become a dispensing-only practice, download the Dispensing-Only Practice Form (Word: 237KB) and the bank mandate form (PDF:431KB).

Complete the forms and return them to nhsbsa.pao-contractadmin@nhs.net.

Terminate your GOS contract

Under the terms of the GOS contract, you can request to terminate your contract. 

Your contract will be terminated 3 months after you serve notice, or at a mutually agreed date.

If you are part of the London and South East regions' POC and want to terminate your contract, download the Termination form (Word: 256KB).

Complete the form and send it to nhsbsa.pao-contractadmin@nhs.net.

Contract Variations

South East region

If you're based in the South East region, we are processing the following variations:

  • additional premises
  • change in GOS hours

To deliver ophthalmic services at a different or additional address, you need to download and complete Annex 8.13: Mandatory services variation to provide ophthalmic services at a different or additional premises form (Word: 35.8KB)

You should return the form to nhsbsa.pao-contractadmin@nhs.net.

If you want to change your GOS hours, email us using: nhsbsa.pao-contractadmin@nhs.net.

London region

If you're based in the London region, we are also processing the following variations:

  • relocations – mandatory (note that this does not include additional premises as this would be a new application)
  • change of address – domiciliary practice
  • adding a new borough for a domiciliary provider
  • adding additional premises to a mandatory contract
  • changes in company name
  • change from an individual to a partnership
  • partner joining or leaving a partnership (not to or from an individual)
  • change from a partnership to an individual
  • addition of directors to a body corporate
  • change to or from a body corporate
  • change in GOS hours
  • change of trading name

If you want to make any changes listed above, then please email nhsbsa.pao-contractadmin@nhs.net with:

  • the change(s) you want to make
  • supporting information (for example, if it’s a change in GOS opening hours, provide us with the old opening hours as well as the opening hours you want to change to)
  • your current trading address
  • your contact number
