We've produced a document to provide GP IT suppliers with a list of the corresponding SNOMED codes for the HRT drugs which should be automatically placed on a separate prescription. A formal list of drugs covered by the HRT PPC is available in the Drug Tariff Part XVI Section 10 - contraceptive services, sexual health related treatment and HRT PPC.
