Patients who have recently visited a dental practice in Wales might be randomly selected to take part in a questionnaire. This is because the Welsh Government and Public Health Wales want to gather feedback from patients about their NHS dental care experience.
We'll contact the selected patients on behalf of the Welsh Government. Patients do not need to do anything unless they're contacted.
Communications toolkit for dental practices and local health boards in Wales
We've produced a communications toolkit to help dental teams in Wales communicate the importance of patient questionnaires. The toolkit includes:

- printable posters
- digital waiting room screen image
The resources in the toolkit are in English and Welsh.
