Forms you need to manage a member’s pension record.


Employers should use Pensions Online (POL) to produce an estimate where possible.

If you don’t have access to POL or you need a manual calculation, you can use these forms:

Protection of Pay

For individual members:

Use the request to protect Pensionable Pay (SM R9 EA) form (Word: 47.6KB) to request individual protection of pay for a member.

For several members:

If you have a number of staff who have suffered a reduction in pay through no fault of their own, you should complete the universal spreadsheet (Excel: 144KB).

Make sure you password protect the spreadsheet before emailing it to NHS Pensions.

Use the universal bulk reply letter template (Word: 40.5KB) to notify members.
